Outpatient Program Eligibility


The Assurance Health & Wellness Center serves adults and children seeking primary care and/or behavioral health services. This includes individuals with the following types of health insurance: AHCCCS, Medicare, most commercial plans and those who self pay.

Individuals who are enrolled with Cenpatico Integrated Care and struggle with the challenges of serious mental illness may also be eligible for dental services, supported employment, supported housing, peer support services, and an intensive 9 week wellness program focused on managing chronic disease.

Eligible individuals (based on health plan benefits) have access to a nutritionist, dentist, and pharmacist who aid in identifying the best course of treatments for each individual.

Getting Started-Wellness Program

Based on our integrative care model, each person seeking care will be offered primary and behavioral health screenings. This will include a health risk assessment to identify current health conditions as well as screenings for depression, substance use and anxiety. Person centered planning with a case manager or recovery support specialist will help each individual reach their health and recovery goals, increase independence and achieve self-management of their health and well-being.

Our Classes:

  • Stress Management: components of the Relaxation Response by the Benson-Henry Institute for Mind-Body Medicine at Massachusetts General Hospital will be implemented. Yoga and meditation are two ways by which we will facilitate relaxation in order to decrease stress.
  • Healthy Eating, Physical Activity and Restful Sleep: Will be addressed through 1:1 nutritional education and consultation with a PhD nutritionist as well as classes on healthy habits. Classes will be sensitive and realistic to a member's financial situation, living conditions, accessibility to healthy foods, and other environmental factors.
    Diabetic Teaching and training will include utilization of the program Journey for Control, developed by Merck to teach through manipulative and visual aids the types of diabetes, how to control/prevent diabetes, medications, and lifestyle changes. The program will be taught by certified diabetic trainers that include physicians, RNs, and LPNs.
  • Physical Activity: will not only include access and use of our on-site gym, but instruction on how to incorporate physical activity into a member’s life.
  • Restful Sleep: Members will participate in classes on sleep hygiene and the importance of sleep and its role in good, overall health.
  • Service to Others and Support Network: members will learn through their support groups the importance of human connections which is what forms the core of WHAM. Peer support and self-help are essential in recovery. Social support is the most critical factor when it comes to how people handle stress and stressful events. Being able to share experiences allows members to feel they are part of a supportive group that ultimately will allow them to meet their goals.
  • Maintenance of a Positive Attitude and Cognitive Skills to Avoid Negative Thinking: During all classes and support groups and throughout the program, members will be taught to identify positive things in their life, how to increase and facilitate a positive attitude, all of which fosters hope. Hope and a positive attitude are related and research shows the effect of positive attitudes and its results on one's whole health and resiliency.
  • Spiritual Beliefs and Practices and A sense of Meaning and Purpose: Members will be able to access a 12-step program, SMART, and faith based meetings in order to facilitate recovery.
  • Health Risk Screenings: We will utilize a Health Risk Assessment (HRA) that will include identification of the following risk factors that are defined as Metabolic Syndrome, which is correlated to an increased risk for developing Type II diabetes and cardiovascular disease.
